I see that as an option, but I can't get it to work. It is grayed out when I have "Numbered" set, not surprising because that doesn't allow much editing. If I turn Custom on, it is available, but then none of my in-text citations are numbered. I just get empty brackets. So it seems that th...

Just found the alphabetizing option instructions in the manual (p. 194). However, that alphabetizes all the authors in all citations. Is there anyway to do that within each citation group?

I have made up an NIH Grant Format (PHS 398) that I'd be happy to upload or share. It follows the recommended guidelines, including the most important addition that it includes the PMCID and PMID. It also doesn't print those fields if they are empty, which is possible in older citations.
