SOLVED THE PROBLEM.
The author field was, indeed,
Australian Commission on Safety
Quality in Health Care,
INCLUDING the newline at the end of the first line - which I guess told Bookends there were two authors.
So, removing that now gives me:

I had indeed selected a format for the citation. Once I reverted to "Content", all is well.
