I'm not sure I remember exactly now. I think I used a larger (15 MB or so) cache, and was able to open the database by setting Reference View as the default in Preferences. Then I selected All references (made them hits), exported to a Bookends file, and then imported that file to a fresh database ...

Sorry, I disagree about the utility of Spotlight for Bookends.
Clearly it currently makes no sense for large monolithic files. However, Spotlight 2 will have the ability to search them.
