It's entered in the temporary citation. Since you're using Mellel, double click on the citation object and enter the text as I showed (you won't see curly brackets in Mellel). In theory there is no limit, which is why I can't think why this should happen. And it's tough to debug from a distance, especially since I don't have your 100's of PDFs. For citations don't output the p- (unless that's what you want). To cite ad hoc pages, use Cited Pages

You tell Bookends what to output before and after the Cited Pages in the format. So edit the format and remove the ", " in the Before field.

Change the default attachment folder to something else. Then, add one attachment from the folder Other's Papers. Bookends will now treat Other's Papers as an ad hoc attachment folder. Impossible to know what's going on when that happens. If you can isolate which reference is causing the problem and I can reproduce it it should be possible to track it down.
