ok - i guess that's how it has to be.... but it either 1. causes a huge buildup of duplicate pdfs in my attachments folder, or 2. substantially increases my work effort by not allowing me to simply grab all the references (with pdfs) that pubmed pulls down and then use "remove duplicates" to get rid ...

How do I use a "proxy" server?
I can login to my university's library webpage (beckerproxy.wustl.edu), supply my username:password, and from there search Pubmed (via a web browser) with access to all PDFs through our subscriptions.
I can also login to our VPN, and then fire up Bookends to search pubmed and download all PDFs ...

largest usable database?
I've got my internal cache set to 200mb - I've got 1gb of RAM... is there a limit to the size of the database that i can have? i currently have one with ~40000 references, and it's 195mb... it seems like it's getting a bit large for the program to deal with effectively...
