I ended up resolving 1 as I note above; it seemed like the presence of articles (a/an/the) was affecting the capitalization of the word following the article. I thought I'd let you know for future reference.
For 2, one fix might be to add ( and " as possible ...

I'm having several title case problems that I hope I can get some advice on.
2. I have a bunch of titles in Chinese characters. If the texts came with their own English titles, I write:
ChineseChinese. EnglishEnglish.
But if I am giving my own translation, I include parentheses:
ChineseChinese ...

I have files that are primarily in English but have some Chinese characters in them. When I "scan document" with BE, some (around one third to one half) of these Chinese characters disappear; they are replaced by an underscore (_), and I have to go through and retype them.
