Word 2008: lost style after scanning

I'm writing my report in Word 2008 and use numbered headings with certain styles.

I found that most of the numbered headings disappear after I scan my document for references (Bookends 10.2). This also affects my table of contents and styling of the body text.

What can I do?

pdf viewer

Does anyone use Skim as the pdf viewer and make notes when you read along?

How could I view the notes (e.g. highlight, boxes) that I made to the pdf file using the pdf viewer in Bookends?

Does this mean that if the reference in the db has no URL or doi, then WOS search won't recognize the reference as if it's not in the db?

Recognition problem with different search engines

Some of my saved references are n...

Search error code -39?

SQL Error

I use PudMed to search for a reference, and then I'd add the result to my library. When I try to add using the "Copy To..." and choose my library, there is an error box saying "SQL Error: line 1:1: unexpected token: DESC". The newly added reference wouldn't show up in my list rig...
