I appreciate the clarification. Changing Create Comment to Add Text really does help distinguish the action. Can we change the author for annotations and markups?
The author for annotations and markups to PDFs defaults to the full user ID. Do we have a way to change this? For comparison, Preview and PDFExpert give a global Setting to define an author. Acrobat forces you to change the author on each annotation one-at-a-time (ugh!).

I am also often confused by the variations in terminology --> comment, note, PDF note, and note card. We also have an option to "quote" selected text as opposed to selecting note card. Respectfully for due consideration, as you think about possible improvements, I would have an easier time...

I prefer a way that will allow me to display only the count (of references) on groups and not the count (of sub-folders and groups) on folders. So, yes, I prefer also the fourth option.

I find little to no use for folder badges. I would be pleased to have the additional option in the context menu to Show/Hide Group Badges. Yes, the macOS version of Bookends flattens the entire Attachments folder. It is entirely unaware of sub-folder locations to distinguish files. Hence, you cannot have a working Bookends library contain both of these files at the same time ...Attachments/subfolder1/Test.pdf and ...Attachments/subfold...
