Ah ok, so how can I tell which of the three options is in use?

There aren't three options, just one (set). One "et al." text, one "in italics", etc.

You can have this set of options applied to Authors, Editors, or both. For example, you can tell Bookends to use et al. for authors but not editors.

How/where do we define a format to edit the output? I am only seeing the option to use other pre-defined outputs. I would like a_d_t as well.

That's done in the Format Manager.
