PubMed MeSH terms and keywords in BookEnds

Dear Jon and others,
I've seen previous discussion about MeSH terms and keywords, but I am wondering about the mechanisms by which Bookends extracts the MH field in the PubMed import filter. It seems that it just takes the MeSH major topic rather than the whole term? For example, I searched PubMed using "Inosine/pharmacology"[mh:noexp], but the Bookends keywords only contains Inosine. Is there anyway to get the whole MeSH term in the keywords field, or at least the pharmacology subheading included in the keywords? Is that something that happens on the PubMed side or how you've coded Bookends?
Thanks,
Todd
