out of bounds error when I scan a document

I have a sudden problem with my thesis, which is causing an out of bounds error. Bookends seems to verify the citations ok, asks if I want to generate a bib, but throws an error before formatting citations, and before my WP (Mellel) replaces any citations.

I have no idea what I have changed since it stopped working, so if anyone has any clues, I am desperate to fix this ASAP.
I have rebuilt the db, and also tried earlier versions of the doc (which scan ok). Earlier versions of the db give same error, so I believe it is a problem with the document.

***Update - I just deleted ALL citations from the doc - almost the whole thing, so there are no citations in the bib palette. It still scans through a really long list "Looking for..." then gives the error again.
