I’m sure I’m doing something stupid, but can’t figure out what it is.
The screenshot tells the story:
Although the comma after a title overrides the command to format the title in Title Case, in this one format the comma itself shows up as part of the output.
(Of course the comma shows up in the formatted output, not only in this example window.)
Here’s what’s entered as Title for this reference:

I get the same result in a new database.
Your screenshot has "Title: as entered"
I need to have "Title: Title Case" for most entries, which necessitates using the comma to avoid Title Case for foreign language refs.
I thought it was the particular format (one I assembled to satisfy an editor) that was the cause of the problem.
But I now see this:
If the formatting code calls for a comma to follow the title AND the title entry for the reference is followed by a comma (to override the Title Case command), the unwanted comma shows up in the output.

The style guidelines I have to follow call for a comma after the title in the bibliography, along with a mix of titles in Title Case and others 'as entered'
At present, I find no way to do this.
A local phenomenon? User error?

I set up my format to be the same as yours, and I still can't reproduce this. Here's what I get:

Updated to 12.2.3 and the problem has disappeared.
