Triplicated References?

Hi - could anyone give me some advice.

I've just started using Bookends (9.0.

so I may be doing something wrong but when I scan my document, my placeholders all get changed into triplicated references. Here's an example of the final output.

(Kline and Bajandas, 2004; Kline and Bajandas, 2004; Kline and Bajandas, 2004)

I'm using Mellel 2.1.2 as my wordprocessor and the temporary citations are not duplicated before I scan the document.

Any ideas whats causing the extra copies of the reference to appear?

Any help would be appreciated.
