I agree with the suggestion for a modifier to bypass title case in particular circumstances (eg non-English titles). This may also solve the problem I am having with this situation:

For me, usually, in a title, the words 'the' or 'a' ought to remain in lower case. After a colon, however, they should be in upper case. This does happen currently, but not if there is a quotation mark in the way. eg: <Title of Book: "the Title">. Is there a reason for this? This wouldn't be an issue if the ignore list was case sensitive, which it doesn't currently appear to be - so I could put the first letter capital versions in that and then if I type them in the title field, I can rely on it not being overridden by the title case process.

A related problem is that of titles within titles: for some formats they need to be italicised and for others they need to be in quotation marks. Is there a possibility of introducing some kind of tag that could be put round a title within a title, instead of introducing ad hoc formatting into the title field, which is what I am currently doing?

There are always going to be exceptions that are tricky to handle. I don't think a markup language embedded in the title text is the answer, though. I'll see about capitalizing the first character after a colon AND a quotation mark.
