I have an author whose name I mistyped, so that it looked like she had middle name of McVay. In the citation this gets initialised as M.V., so that she ends up as Surname, F.M.V rather than Surname, F.M. or Surname, F.McV. (depending on preference).

I've corrected my typo, but I do know people with McMiddle names, so perhaps it would be good to look at this?

I don't know if I am introducing something too different here, but this thread reminded me of my problem when formatting with SmallCaps surnames like McCann, which results in something like 'Mccann' (first capital, following smallcaps). To avoid this I had to type 'Mc Cann' (separated), but then I have to control all such cases and delete the extra space.
Would it be possible to make it automatic with McSurnames?

That's something very different (small caps are a royal pain). I've fixed the current problem with initials and names with multiple cases for the next release.
