What's the best to deal with references without authors? I.e. references should be formatted differently when there are no authors.
If there is at least one known author, the format should be...
Author(s). Year. Title. Rest of information.
If there is no known authors, the format should be...
Title. Year. Rest of information.
Another problem are the citations in the document. In the first case they should look like (Smith, J. 2008) but in the second case like (Title of book 2008).

I suggest you create a new Type -- No Author -- or something like that. In your format, for that new Type arrange the output accordingly (t. d. ...).
You'll also want to create a custom citation format, which is called by your basic format, to create the citations. Again, in the custom citation format tell Bookends to output

when the Type of No Author is encountered.

The first field in a type (in Preferences) seems always to be 'a' in Format Manager. If the above doesn't work, couldn't I just name the first field in my No Author type Title - and use (a. d. ...) for reference format?

You can do all that in custom formats. See APA 5th as an example of some of these.
The first field in the vast majority of formats is "a" because they begin with authors. Offhand, I can't think of one that doesn't. But Bookends doesn't care. You can begin a format with "t" just as well and the title will be output. Your custom format for a no author reference would begin

It is of course possible and not unusual, to use "Anon." as an author name and refer to "Anon., Year" in the text which is probably easier than using titles.
