Unmatched citation error

I have scanned a document, built the bibliography, then had to revise the information and add in one more citation. No matter what I try, the citation comes up unmatched. It list the reference in the unmatched window, when I highlight it and click on done and rescan it still comes up unmatched. Can you please help. I am using Word 2004.
