MLA still doesn't work

Among the new features of 10.5.3:
ยท Disambiguated MLA citations using the short title will enclose it in quote marks if the reference is a journal article or book chapter but not if it is a book.

book short titles for disambiguation should be italicized or underlined for MLA. As a general rule, the disambiguating short title takes the form of the title's appearance in the bibliography. This should be fixed, or you should expose this to the user for modification.

If you italicize the short title in the reference, it will appear in italics in the disambiguated citation.

Is that really the best solution? This would require my going back through 600-700 references to italicize the short title... Why can't this be accomplished in the style? Am I the only user of this style in Bookends? Because anyone who wants to use Bookends to produce MLA citations will face the same problem.

It's the only solution for now (and of course you just need to do this for a few references, books that you cite that have to be disambiguated. That's unlikely to be hundreds. Adding the italics automatically of course can be done, but it would be difficult to implement. Since there doesn't seem to be a lot of demand for it and there is a workaround, I haven't given this a very high priority.

In the next update, Bookends will automatically italicize the short title of a book when it is used to disambiguate MLA citations.

Even since Bookends 10.5.5 dropped, short titles still get output in quotation marks, and are unitalicized, in MLA textual citations in which disambiguation is necessary. Do I need to do something to get rid of the quotes and make the italics happen?

I assume you are using 10.5.6 now. As of 10.5.5, short titles should be in italics. If they're not for you, you should contact support with examples.
