Formatting details

Hi...
I'm having trouble with some formatting details. Here's one of my formatted citations (in my
customized "Book Chapter" format:
Timothy P. Weber, “Fundamentalism,” in Dictionary of Christianity in America,
ed. Daniel G Reid, Robert Dean Linder, Bruce L Shelley,
and Harry S Stout, (Downers Grove, Ill: InterVarsity Press, 1990).
My questions:
(1) I don't want the comma after the last editor's name... But I can't figure out where it's coming from.
This is the setup for my primary order:
a, “t,” $in $v, $ed. $e f `(`l: u, d$; reprint, $u12`)`.
I thought that maybe the comma was generated by Bookends' thinking there was
yet another editor -- but I've checked, and there are no returns in the reference data
after the name of the final editor.
(2) I'd like periods after the middle initials of the editors. I've included those periods
in the reference data, and Name Punctuation is set up (with Editors same as authors) like this:
After initial: . [period]
Between names , [comma space]
After surnames , [comma space]
Between last names: and ^, and
