Format available?

Hi Jon,
I have to format a paper for a monograph and I can't figure out how to format my data accordingly. Is there a format available for this? Or can you indicate how to create one?
The publisher wants intext citations quoted in footnotes with "AUTHOR (year)":
I have no problem to do that with a custom citation format.
In the bibliography they want the intext quotation repeated followed by a colon and then by the reference with full author name.
I can't figure our how I can put the author name twice into a format, once in "ALLCAPS-only-surname" and a second time in "TitleCase-full-name"? Is there a way?
