New to Booksend. Giving it a try.

When I ask BE to rename and attachment, and then go to the attachments folder, I find that the names have not been changed.

Zotero has a command that renames the attachment using the information in the bibliographic record with which it is associated. Is this possible in BE?

Yes, Bookends does the same thing (renaming rules are in Preferences, Refs tab). You seem to have lots of problems with things that involve the Finder, probably because you're using Finder substitute. Please try these things on a standard Mac install. If you have problems there, contact tech support for help (support@sonnysoftware.com). The forum is not a good place for tech support
