Web links in bibliography?

Since my bibliography is going to appear on the Web, I would like the citations of Web pages on it to include links to the pages.

Does anyone have a format that does this?

Or how could I build one that does?

Please check out the section in the User Guide (Help menu) that deals with Bookends Server. There are examples there of how to include a hypertext link (if the reference has the URL) in a format.

What is your preferred output format (pure HTML, use a CMS)? I use LibreOffice (I wanted clickable links in the exported PDF), and there is no way of using Bookends to make the links "live", so I made a macro that takes <http://dx.doi.com/stuff> in an bookends formatted ODF and converts them into clickable links, I suspect you can then export to HTML...
