Imported files attach incorrectly?

Using 12.8.1, I'm finding that adding a file to the watch folder sometimes attaches that file to an existing reference, sometimes causes a previously imported reference to disappear, and sometimes replaces an existing attachment. See attached screenshot for an example. Note that the second file in the library attachments folder no longer displays its reference in the Watched group. I've rebuilt the library file, but the problem persists.
