Syncing problems, lost annotations

I've recently run into an issue with Bookends iOS while syncing to Bookends for Mac. I am running the latest update of Bookends for iOS, and Bookends for Mac is version 12.8.4. I am currently running OS X Yosemite (10.10.5).
When syncing my iPad over WiFi (so no cloud service) to Bookends for Mac, the highlights and annotations completely disappeared from the PDF. The PDF on the Mac shows zero highlights or annotations, and everything disappeared from the PDF on the iPad as well. When opening the PDF on the computer with Preview, I can confirm that there are no highlights whatsoever. My workflow in this instance was as follows: I had found a couple of articles that I wanted to read later, so I created an entry on Bookends for Mac but didn't attach a PDF (my university library's website was bogged down at the moment, so I decided to wait and download PDFs later). Later, I was on Bookends iOS at a bookstore, downloaded the PDFs and sent them to Bookends from Safari. I then spent the day highlighting and annotating several articles. When I returned home and synced over WiFi to my Mac, ALL the highlights and annotations were gone. I have tried syncing again, shutting down my iPad and resyncing, restarting Bookends for Mac, and all to no avail.
While I understand at this point that the research is lost
I would like to know if the error is a bug in Bookends or in my workflow. Does adding a PDF directly to Bookends iOS, and then highlighting/annotating that PDF, result in the annotations being lost if I haven't synced the PDF first? In other words, do I need to sync a plain PDF (no highlights or annotations) between Bookends iOS and Bookends for Mac BEFORE adding/syncing any highlights or annotations?
I even tried re-highlighting certain sections of the PDF on my iPad, syncing again, and the highlights still disappear. I tried highlighting the PDF on my Mac and syncing, and those highlights then appear on the iPad---after that, it seems like everything syncs the way it is supposed to, with annotations finally syncing from my iPad back to the Mac. So I guess what I'm asking is if this is a problem in my workflow, or a bug that needs to be addressed. I just want to make sure that no future research is lost.
