Same author and year problem in APA

I'm new to Bookends. I'm using APA and have a problem with citation of papers by the same authors in the same year. Such citations should be identified with a letter suffix (e.g., Smith & Jones, 2000a, 2000b). However, in a paper I have just formatted, Bookends is giving me:

(Smith & Jones, 2000a, 2000c)

It seems to have skipped the letter "b" in both the in-text citation and reference list. I only have two papers by the same authors in the same year.

Any suggestions?

This is a tech support question. Please contact me directly. It would help to have a small database, document, and the format you are using (zipped) so I can try to reproduce it.
