No, Bookends actively defaults to the top of the PDF. I've had occaisional reports of this over the years, but I've never been able to reproduce it. Please try another Mac, if you can, as a diagnostic.

No, you can't use the iCloud Folder for iOS Sync between Macs -- as I said that folder is created by iOS). It's easy enough to switch attachment folders in the future if you need to. Yes, you'd select Other and then your folder in the iCloud Drive (which you can indeed use with Cloud Sync). Note that if you use Bookends for iOS those PDFs won't be synced with your iOs devices. I'm not sure what labels you're referring to, maybe color labels? I'm speaking of PDF tags, which can be quite powerful and have the advantage that the values you set belong to the PDF itself and can be used by the Finder and other apps that understand PDF tags. 1. I'm not sure what you mean by assemble, but if you mean some sort of automatic mechanism to find PDFs that have annotations and display them, there is none. It's there, you've made the edit pane too narrow to display all of its contents. Make it wider (drag to the left) and you'll see the color label control again.

There is no preference for copying an attachment to it's own folder. When you attach a PDF a dialog offers the possibility to Move or Copy the attachment to the Bookends folder. I don't know if it helps your workflow, but did you know that you can switch edit panes using the keyboard? Command-2 is the Additional Fields pane.
