a. Make sure the attached pdf (or whatever) is deleted when a reference is deleted from the database (perhaps optionally).
b. Manually delete an attachment from the reference window (not just the link)
c. Search for and remove unlinked attachments.
Is this possible?

No. And it is complicated by the fact that the same file could be attached to other references.

Kind of. Right-click on the attachment (in the Keywords field) and choose Reveal In Finder. You can then do whatever you want with the file.

However, I didn't find this feature by myself, perhaps because the attached file is often scrolled out of view in the keywords field (because of all the, well, keywords in there...). Would it be possible to add a "Remove attachment" command to the menu brought up by the little triangle under the "Attach" button"? If there is more than one attachment there could be a submenu with all the files listed....
Or, alternatively, to use a dedicated UI element for managing attachments entirely. Storing them in the keywords field doesn't feel all that intuitive, really. At least to me, the managment of pdf's is a fantastic feature that I have come to rely heavily on. Perhaps it could be more emphasized in the UI?

This is tied to the issue of more fields. I personally find the attachments in the keywords field to be fine (but of course I would, since I designed it). I suppose a remove attachments option could be added (but really, how often do you want to remove attachments you've made)? Do you mean "delete attachment"?

Yes, sorry. Sloppy use of words. Delete is what I meant. And normally I don't see a big problem with using the keyword field for storing attachments, as I rarely need to really look at the file name in there.
However, I have recently had a few occations when I wanted to attach a new version of a file to a reference (for example where I previously just had the proof attached, or an "in press" version of a manuscript). Of course, in such cases I want to get rid of the old version.
