Is there a way to make a smart group (without SQL) only include those references that have an attachment? (Or does that require using the SQL feature)?

You'd need an SQL group for that. But do you know about virtual groups? If you click on your smart group and then Command-click on the Attachments group, Bookends will combine the searches, giving you what you want. Make sure that you've set such groups to use AND logic, so you only get references that the smart search finds AND have one or more attachments.
