The plus sign is at the bottom left of the list of formats. I'll include a screenshot in the next version of the User Guide.

The way you added the relationship was incorrect. Look at the entries that ship with Bookends -- there is only one relationship for each. Bookends will automatically offer it and the inverse in the popup menu. You actually created two new relationships

I may not have understand your comment about what is problematic. I think now that you mean the Global Change operations I suggested. That's the first I've heard of that, but if EndNote database becomes corrupted it's definitely an EndNote bug. Bookends simply sends AppleEvents to EndNote to export the references and EndNote XML. How are you importing the EndNote XML? If you are using File -> Import References -> From EndNote and Bookends can locate the PDF, it should be copied to the default attachment folder, in a subfolder called EndNote Attachments. Please refer to p. 321 of the User Guide (Help menu) for how to configure the Summary View. Then read my last post again.
I wouldn't add autocomplete to the group search, but I might make it a live search, which filters the results as you type.

A couple things to note, however:
1. When an item is selected that is part of a group, and the folder is closed in the Group Pane, it's not the case for me that the folder shows highlighting.
2. I'm not able to seen any information about groups in the Summary pane. Maybe I'm missing the setting ...

I don't know either. Nothing has changed in Bookends wrt the watch folder for quite some time. The behavior may be due to latency or other intermittent network issues, changes in the way Google Drive deals with the file, or something else. That's how it has always worked. They are imported when first dragged in. The advice above applies only when there are files in the watch folder when it is first enabled -- those files will be left alone because the "added to folder" event never fires. No, Bookends can't do that. Not even Preview does that. There are dedicated apps that can, however, like Adobe Acrobat Reader (Edit Document Properties). You can google for other apps or utilities that let you modify a PDF's metadata.
